Tools from each module should be used (not on each client – but overall) |
| Session Date | 10/10/2024 |
| Session Number | 15 |
| Total Session Minutes | 90 |
| Homework assignment to client/group | Journaling prompt response. Kirtan kriya-12 minute version, PT exercise with breath, Taking in the Good, at least 3x/day, continue exploration of a positive heart felt sentiment to counter her negative messages, keep up gentle yoga classes. Complete the WHOQOL-BREF questionnaire again, bookend to session six. |
| Activities | Check-in, center, ground. Pain=0 in back, 2 right hip flexors; fatigue=3. Awareness practice. Selection of affirmation and color cards. Discussed details of some intake information to confirm and make further inquiries to make sure Capstone presentation is accurate. Learned more about her depression and the circumstances surrounding her in-patient care for major depressive disorder (MDD.) At the wall: down dog, plank up dog sequence repetition. Introduced a new Warrior II, triangle, side angle sequence at the wall. Sun breaths. Prone: sphinx, seal (PT exercise.) Guided relaxation. Talked about client’s progress with and strategies for creating a counter narrative to her persistent negative message/mantra. |
| Client/Group progress summary | Client shared that her neurologist informed her the “growth” she mentioned last week is a meningioma and it is benign. She will consult him regularly to assess brain aneurysm and meningioma. She is pleased to have practiced the Kirtan Kriya for 13 minutes. It is now calming, restful, and meditative for her. Her low back pain is gone, and she is having pain in what appears to be the hip flexor area on the right side. She may return to the yoga studio where she took hot power yoga classes and take a restorative yoga class to rejoin the yoga community there. That seems like a sign of acceptance and resilience to me. |
| Reflection and self-evaluation | The client is working hard to counter the persistent negative self-talk in her mind. She knows it will take some time to chip away at these decades old patterns and is ready to do just that. She also knows that the affirmation she creates needs to be genuine, even though she may not believe it each time she repeats the words to herself. Encouraging. I feel as though this client has given me the opportunity to utilize a broad range of the skills I have learned throughout IYTH training while keeping a consistent care plan, adapting emphasis as more information was gleaned from the client. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | Discuss client’s progress with crafting a positive counter-narrative, kirtan kriya, dynamic Warrior II asana linked to expansive breath practice. Include some psoas lengthening As per week before, explore her desires for her life, Upanishads “You are what your deepest desire is. As is your desire, so is your intention. As is your intention, so is your will. As is your will, so is your deed. As is your deed, so is your destiny.” Discuss plans for future sessions that have been scheduled through the end of 2024. |
