Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Three-part breathing daily for 5 minutes followed by alternate nostril for 5 minutes
2. Kirtan Kriya daily 5 minutes
3. Positive affirmation daily and as needed
4. Gentle movement with breath 15 minutes, twice a week and when feel stressed and overwhelmed (stress started with Vignanamaya kosha, but to reduce stress, need to start with the physical body to manage it).

The client has been diagnosed with chronic inflammatory response syndrome (CIRS). She has low energy and is fear of losing cognitive function.

Client/Group progress summary

The client had a disappointing week. She could not manage it very well and was depressed. However, she slept well; perhaps because of not drinking alcohol (we discussed alcohol consumption can impact sleep). She has hurt her right knee but is feeling better.

Reflection and self-evaluation

The client needs a lot of mental support this week; the session was longer than usual. The affirmation (I am happy, healthy affirmation) did not work for what she had to deal with this week, which is understandable. I asked her to think about other phases that would work for her. Praktipaksha Bhavana can be applied here; replace negative thought with positive one. I had to change my plan entirely to avoid putting pressure on her knees. I also added Breath of Joy to improve her mood and energy level. Overall, the session and yogic wisdom seemed to work well; the client reported the next day that she woke up a happier person.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
