Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Gratitude & shower meditation

Activities

Check in: Client has been doing his HW for the last week 5x gratitude, 3x shower meditation, has been deep breathing and stretching though out the day. He was unable to go to the gym but came to my restorative and gentle yoga class in addition to YT. His low back is sore but much better. He said he realizes that the lack of movement will make his low back hurt.

He's been setting his alarm for 90 min when he remembers and will stretch, walk and breathe when it goes off. He is doing about 10min/day.

I continued with the witness meditation - client was able to sit and meditated for 10+ min. His legs ended up falling asleep so we stood up and did the breath of joy. bouncy twists

Client/Group progress summary

Clients LB was still sore but much better than last week. Client was able to do his HW and has added an extra 5 min on his own. He is setting the alarm when he remembers but at least he is remembering. Very good progress

Reflection and self-evaluation

Will not see the client next week because of my knee surgery. Will see client again 10/18 and then final session 10/25

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
