Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

9-19
I began the session by checking in to see if there were any physical or mental changes. She reported significant improvement overall physically and mentally.
We started with guided meditation for a few minutes and moved to physical practice.
Today’s physical practice included gentle neck/shoulder starch, sun salutations on the chair, spinal twist, lateral stretch, and forward/back bending.
I led 15 minutes of yoga Nidra and alternate nostril breathing to relax her physically and mentally.
10-11
I began the session by checking in to see if there were any physical or mental changes.
Karen shared her experience at her church volunteer yesterday, her friend was at church with her daughter who has a kind of adjustment disorder, a fear of interacting with people. This person had a panic attack in the car on the way home, Karen was able to calm her down telling her deep breathing with holding her hand. She was excited to talk about this to me. She is a nurse and good at taking care of people.
Today I repeated the same practices as the previous one so that she can continue to practice on her own. Today’s physical practice included gentle neck/shoulder starch, sun salutations on the chair, spinal twist, lateral stretch, and forward/back bending.
I led 15 minutes of yoga Nidra and alternate nostril breathing to relax her physically and mentally.
Client/Group progress summary
She has more energy to clean the entire house which I found this is a good way to explain the progress.
She described that she could get into the car normally without pain, something she could not do for years due to pain and her hip replacement. Also, she was able to cut her pain medication twice a day to once a day, sometimes no medication.
Reflection and self-evaluation
Karen mentioned that she is considering attending a yoga class with her friend, I suggested to
check the studio and make sure they offer classes she can follow, because moving up and down may not be easy for her.
This was the last session with Karen, we had only 6 sessions, but she showed a lot of progress.
She learned quickly as she knew the body and conditions she was experiencing as a nurse.
I noticed that her feelings shifted optimistically as we progressed. During the session, she initially said, “I know, I am a nurse!….” but did not hear that after the third session.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session
I will follow this care plan and hope she can practice on the mat for gentle stretch that she enjoys.
I am hoping she continues her practice!
