Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Working on balance, which we just starting incorporating.

Activities

Centering and discussion- discussion on where she is at with herself and her daily practice reconnecting to her goals. Our centering was visualizing those goals while controlling breath
Asanas in chair- Japa, Seated side bend and twists, seated sun A with standing and folding supported lunges, chair pose Downward dog Triangle, warrior 2 Balancing with knee up (chair close)
On Mat- Sufi grind, supported hip opening and twists with bolster and forward folds with strap Supported savasana for yoga nidra
Pranayama on mat, 3 part breath the Ujjayi
Meditation- Deeper connection to self and her goals with visualization on where she has seen herself to where she is now and where she desires to be.
Philosophy- Avoiding Asmita, recognizing her true self, which is deeper than her appearance that she is pure energy. (Atma) Connecting to this energy of self and her truest highest power.

This client is feeling good and stronger. she is able to start working on her balance now that her foot is feeling better and is excited about new progress to watch.

She is beginning to get more and more connected to herself and feels really good about it and is excited to deepen her practice.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
