Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.
Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Client/Group Goals

The client’s physical and mental health gradually progresses towards her goals, i.e., to better cope with daily stress, specifically with hip pain.

Physical level

The pain level reduced significantly after 8 sessions (3 months duration)

Energetic level

The client is moderately rajasic (active). She is still active but becomes more aware of her energy and physical limitations. She rests when it is needed and not doing too much or activities that will aggravate the pain

Emotional

Stress level has reduced through asana, pranayama and meditation.

Intellectual / Sense of self

She identifies herself as being active. Nevertheless, she is more accepting of slowing down.

Spiritual orientation and support plan

Spiritual orientation has not changed (non-religious but connecting to nature). She has very little interest in spiritual growth (based on the intake form). Nevertheless, she uses yogic wisdom to reframe her thinking and dealing with stress.

Additional Information
Feedback received from client/group, anecdotal or written

The client mentioned that reframing and shifting her thoughts about pain is very helpful. She also finds breathing practice and asana to manage the pain and stress.

Sample of homework given between sessions (after initial homework)

1. Five rounds of Deep breathing with positive affirmation
2. Extended exhalation throughout the day, especially when there is an incident that triggers stress response.
3. Pay attention to how she feels physically when being active (golfing or gardening). Take a break as needed, no push through pain.
4. Chair/floor yoga followed by deep relaxation (15-20 minutes): 3-5 times/week
5. Alternate nostril breathing 5 minutes

I would explain the reason why meditation can help managing pain early on.
