Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

I did not adjust or adapt the care plan for this client, except that I did not teach Kapalabhati. The reason is that it might aggravate her anxiety and stress. I felt that she needed grounding practice. Therefore, I focused on breathing techniques that bring the calmness to the mind to reduce stress and anxiety (box breathing, alternate nostril and three-part breath).

Client/Group Goals

The client’s goal was to find pathways for balance during the time of great stress and anxiety. Through discussion of yogic wisdom as well as physical and subtle practice, the client achieved her goal.

Physical level

Muscle tightness related to stress and anxiety has improved. Her sleep quality has improved.

Her energy level has improved.

Emotional

Improvement in positive affect score and reduction in negative affect score.

Intellectual / Sense of self

She non-attached to the idea that she needed to make decision for her friend. She became more intentional about stepping back and letting staff at the assisted living facility and health care professionals assessed her friend’s physical and mental condition so that her friend could make an informed decision.

Spiritual orientation and support plan

Her spiritual orientation has not changed (she is not religious). However, she feels connected to nature and taking walks in nature supports her spirituality.

She found discussions about yogic wisdom helped her reframing her thoughts and shifting attitude.

What you would change with benefit of hindsight

Although the client's mental health improved after the fourth session, it would be ideal to have a few more sessions with client, especially to support her next goal, i.e., eliminate bad habits. The session ended because the client would be travelling for several months with unpredictable internet access (hence, the virtual option is not possible).
