Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Home practice
1. Three-part breathing daily for 5 minutes followed by alternate nostril for 5 minutes
2. Kirtan Kriya daily 5 minutes
3. Brahmari breath as needed
4. Positive affirmation daily and as needed
5. Gentle movement with breath 15 minutes, twice a week and when feel stressed and overwhelmed.

Activities

The client has been diagnosed with chronic inflammatory response syndrome (CIRS). She has low energy and is fear of losing cognitive function.
• Assessment – Neuro QOL forms for cognitive function and fatigue
• Centering – witness practice

• Seated upward arm stretch, lateral bend, and twist

Client/Group progress summary

The client had a good week up to the day of the session. She has been travelling but did breathing practice and meditation while she was in a passenger in a car. Her breathing pattern has improved (easier to take a deep breath). On the day of the session, she was exposed to dust or toxin (while going through things in a shed) and was feeling dizzy and unstable physically and emotionally. She felt that her feet were not connected to the ground; did not know where she was in the world. She felt emotionally drained and tired. Assessments of cognitive function and fatigue showed significant improvement from the first session.

Reflection and self-evaluation

The client texted me about her exposure episode and suggested that we cancelled the session because she was not feeling well. However, I heard the mentor’s voice telling me to let the client know that gentle practice would still be beneficial. Therefore, I let the client know that we could have a gentler session which would benefit her. I threw out my session plan and developed a new one as I drove to the client’s house. Because the client felt unstable on her feet and lost, I thought that restorative yoga would be helpful. While the client was in a restorative pose, I guided the client to observe her physical body, breath and the mind and accept things as they come (mindfulness practice). I also emphasis the sense of grounding, i.e., connection with the ground underneath her. We did Brahmari breath as the mentor suggested and the client loved it. She could feel the vibration throughout the body. She felt better overall after the session.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
