Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

Her psychotherapist said I need to work one concept at a time with CM. Maybe it is because she can be overwhelmed easily. So I decided to work 1 lock and 1 key each session.

Client/Group Goals

Any progress toward goals
Her goals were to calm down her mind and get tools to manage anxiety and depression, and as I mentioned before, we could see progress toward the goals.

Review and revision of goals.
The goals didn’t change.

Report briefly on each Kosha belowProgress toward wellness or worsening reported by the client/group or that you observed in the following areas
Physical level

She exercises regularly at the gym (4x a week) and has no physical limitations. She hasn’t much flexibility. She was part of the Yoga for Anxiety group.
Sometimes she feels pain in the left knee and back after exercises. This didn’t change much, maybe because we had some interval between sessions. She could not have a session during her college exams.

Energetic level

She had a high level of energy, she said that it was too much that she couldn’t handle, very rajasic. We could see changes here.

Emotional

She takes medication for anxiety and depression. She is an overthinker and very unrested. In her daily life, she was feeling calmer.

Intellectual / Sense of self

Intellectual stimulation: reading novels, college studies, and learning about plants.
This didn’t change.

Spiritual orientation and support plan

No spiritual inclinations. She can connect well with nature and pets. No changes were reported here.

Support Plan
Walking in nature can be her way to connect spiritually.
Journaling
Better sleep: Before sleep, she will listen to a recorded progressive relaxation for 10 minutes.
Mantra repetition or affirmations.
Imagery and Sankalpa

Additional Information
Feedback received from client/group, anecdotal or written

She hadn't written anything yet, but she told me these sessions were very important for her and that she wanted to continue.

Sample of homework given between sessions (after initial homework)

Observe breath, and thoughts when something negative emerges and use the opposite
Practice Be friendly to the happy and journal about it (in the subway)
3-part breath

Personal reflection from doing client/group.
Rough estimate of time spent in preparation and follow up documentation per session5
What you would change with benefit of hindsight

I should have spent more time with her.

Questions, problems, areas in which you’d like more support

More support in informed trauma material.

Did you enjoy your service?

Yes, I enjoyed the service. It was more difficult because of mixed relationships, I had known her since she was a kid.
