| Session Date | 09/28/2024 |
| Session Number | 6 |
| Total Session Minutes | 105 |
| Homework assignment to client/group | 1. Three-part breathing daily for 5 minutes followed by alternate nostril for 5 minutes |
| Activities | The client has been diagnosed with chronic inflammatory response syndrome (CIRS). She has low energy and is fear of losing cognitive function. |
| Client/Group progress summary | She has had a good week and has been doing well. She had a big smile on her face when I arrived. The length of her inhale and exhale was 4-5 seconds each; there was no reverse breathing. The client also mentioned that her breathing has improved. Her balance slightly improves, she can let go of a chair for 10-15 seconds (from only let for a few seconds). |
| Reflection and self-evaluation | There is no need to modify session plan today. I quoted Swami Satchidananda ‘You cannot stop the waves, but you can learn to surf’ when we discussed dealing with unpleasant experiences/events with clarity and not let our emotion take over. I also read a paragraph in Letting Go chapter of Jon Kabat-Zinn’s Wherever You Go There You Are book and made a copy of the chapter for her to read later. I think that the client has come a long way from where she was during the first session. She is less reactive to stress and slowly learns to let go and accept what is. Her husband was making a lot of noise in the next door while we were in session. I was pleasantly surprised that the noise did not distract her, and she was completely still during savasana, pranayama and meditation. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | Continue to improve balance, flexibility, and strength. |
| Report briefly on each Kosha below | Progress toward wellness or worsening reported by the client/group or that you observed in the following areas |
| Additional Information | |
| Personal reflection from doing client/group. | |
| Notify Mentor? | Do not notify Mentor (choose if you wish to continue working on this entry later) |
