Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Daily practice:
• Three-part breath, Box breathing or alternate nostril 5 minutes followed by 10 minutes of gratitude meditation or focus on the breath. Incorporate breathing throughout the day
• Establish bedtime ritual – instead of playing video game until the time to go to bed, set aside 15-20 minutes to breathe and meditate.

Activities

The client was diagnosed with non-Hodgkin lymphoma and completed six rounds of chemotherapy in July. His goal is to use yoga to support his health, mind, and body connection. He is active (exercise at the gym regularly). Based on my observations during the first session, the client does not have physical limitations and has good energy levels. His source of suffering is his fear for the future, i.e., the possibility that the cancer may come back and how this will affect his family; He is in his 40s and has two young children. The asana practice focused on connecting the movement and breath which is one aspect of mindfulness practice (being in the present moment).

Yogic wisdom - Santocha, Isvara Pranidhana, Pratyahara and mindfulness. We discussed contentment; being content with where he is at this moment and not worry about the past or the future. He is doing everything that he can control by taking care of his health - good diet and exercise. The rest is to surrender to the higher source. Pratyhara is for developing bedtime ritual for better sleeping habit.

Client/Group progress summary

The client is doing well. He mentioned that his sleep could be improved. He usually goes to bed late (playing video games until midnight) and wakes up around 6.30 am on school days and later during weekends. He was a shift worker, and he thinks that this may impact his sleeping pattern until now.

Reflection and self-evaluation

We recommended the client to establish bedtime ritual; stop playing video games an hour before going to bed and set aside 15-20 minutes for breathing practice, yoga nidra or meditation. The client is active and enjoys walking; I suggested that he tries walking meditation, if sitting meditation is challenging.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Check in with the client on his bedtime ritual and how it impacts his sleep and overall well-being. Asana will be gentle flow that connects breath and movement to quiet the mind.
