Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Meditation – 5 min daily
2. Breath – Three-part breathing followed by alternate nostril 5 minutes daily
3. Gentle yoga – 30 minutes once a week.
4. Continue home practice challenge – pick a habit that he/she wants to cultivate or eliminate and practice for 2 weeks or a month. Bring yogic teaching to daily life.

Client/Group progress summary

All clients show significant improvement in their balance; everyone can stand on one leg and tandem standing for at least 30 seconds. Breathing pattern is good. The participant who had reverse breathing is doing better; her breathing pattern has improved (breath into the abdomen and then chest), although the length of her breath is not as long as others. She said that she has practiced at home and is feeling much better (calmer and feels that she is getting more air). Everyone has been doing their home challenge, although they found it difficult at first, but over time the practice becomes easier. One participant worked on meditation and on not being reactive to news or things that normally disturbed her. One participant practiced compassion toward her elderly mother-in-law. One participant practiced eliminating his road raging habit. One participant practiced letting go and the last participant developed a daily ritual. All feels the benefit of the practice – they are calmer and less reactive to a situation.

Reflection and self-evaluation

The participants enjoy the series and think that yogic wisdom is immensely helpful. I am happy to see the improvement in all koshas. The home practice challenge works well, and it is helpful for participants to listen to other experiences. Overall, the series went well; I will offer it again in the future.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
