Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Client/Group Goals

AC’s goal is to improve her overall health, relax her mind, and increase flexibility.

Physical level

AC is presenting a problem is elevated stress levels due to her daughter’s health issues and her own health issues. Also, she expressed chronic pain in her lower and upper back, neck, right knee, and right foot.

Energetic level

AC always looks calm, and she does not show much facial expression, it is not easy to see the difference in 3 weeks.
At the last session, she stated she felt to have more energy in the morning.

Emotional

I stated below that she now AC has new hobby and activity which is patchwork. Engaging detail-oriented patchwork increases awareness of mindfulness and focus. And while she is working on patchwork project, she can temporarily escape from stressful situation. This is good practice for her.

Intellectual / Sense of self

My mental/emotional care plan for AC was to find a community group that aligns with her interest, and she can join. The reason for this is that it will provides her with a sense of belonging and connection to others who share similar interests, it will help to reduce stress by enjoyment, engaging in hobbies, also she can have some goal.
She reported that she recently started attending a patchwork group, she said that this was once a month class but she really enjoy meeting others. I think engaging in this activity will provide her enjoyment of learning and goal setting. Ultimately it will contribute to her personal development and increase sense of fulfillment.

Spiritual orientation and support plan

AC just started a gratitude journal last week, but I haven't seen the effectiveness of it yet. I will be seeing her two more times, and I am hoping to receive positive feedback from her and I hope she continues to practice this activity.

Additional Information
Feedback received from client/group, anecdotal or written

AC reported her upper back pain is improved, less tight and stiff as she stretches her neck/should every day, but she still has pain sometimes.

What you would change with benefit of hindsight

I would refine my intake process to gather more comprehensive information from the outset, ensuring a clearer understanding of the client's needs. This would enable me to create more personalized and effective care plans.

Questions, problems, areas in which you’d like more support

The most challenging aspect was gathering information from my client. Unlike Jack or Donna's assignments, I did not have background information on my client during my first session. Assessing Koshas except Anamaya and Pranayama Kosha and creating the care plan was difficult due to this lack of information. This challenge highlighted the importance of thorough preparation and understanding the client better when I start my 1st session with my client.

Did you enjoy your service?

I found the sessions with AC interesting even though I was a little nervous, and discovered about her at every session. However, they highlighted areas where I could improve.
