Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Yogic wisdom – Reviewed Yama and discussed Niyama and during asana, relate back to Yama and Niyama when appropriate. For example, notice how the body and breath feel during each pose, different between the right and left side. Saucha (purity) – positive thought/affirmation during meditation. Ahimsa – not pushing through pain.

Client/Group progress summary

One of the clients fell when she went hiking and bruised her torso. I asked her to be mindful and not push through pain. A client is more comfortable in getting up and down the chair (she was slow in the past sessions). The client who has rheumatoid arthritis did alternate nostril with her left hand because she had pain in her right finger joints. Another client also used her left arm because it is difficult for her to hold the right arm up for a long time, suggesting crossing her left arm under the chest and resting her right elbow on the left arm/hand next time.

Reflection and self-evaluation

I started the class with breathing practice on the floor for three-part breathing practice. I guided the clients to place one hand on their abdomen, another hand on the rib or chest and observe the movement of the hands as they breathed. Supine lateral bend was too much for the client who bruised her torso. Also, she seemed to struggle with movements, although she mentioned that she was fine. So, I modified the class and spent more time on a chair instead of standing with minimal extension of the obliques or bring arms overhead. Seated twist was done gently and emphasized awareness of the physical body.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Joint movements (for arthritis), stabilization of the hip and strengthening the core of lower back pain and balance. Modifications may be required to accommodate the client who bruised her torso.
