Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Call on this "witness" meditation practice throughout the week and continue with movement ritual.

Activities

Centering and discussion- Discussion on her week and how she feels, she expressed she felt that she needed a calmer practice there was a lot going on and she just needed opening movement, stretching and breathing.
Asanas- Chair-Warm hands and wrists sun salutations, side bends, down dog, twists and triangle, chair pose standing, seated chair camel
On Mat- Folds, Leg Circles, Restorative savasana for yoga nidra
Pranayama on mat, Ujjayi throughout practice then Square breathing, alternate nostril
Meditation- Oberserving sensations, thoughts, emotions as your purest self. From the place of presence and shedding layer by layer what doesn't serve you, self judgment, judgment of others, things we can't control and this develops a bright and keen awareness of the truest self evoking inner peace.
Philosophy- The "witness" is the part of you that is aware of thoughts and feeling and emotions, that this part of you can be the observer without judgement or reactions

Client/Group progress summary

This client has progressed in many ways throughout our time together, still needs improving in physical practice but developed in breathing presence and her sense of self.

Reflection and self-evaluation

This client gets body work through massage and has for many years. Her practitioner has been gone for a few months and they just had the first session since we have been working together. Without saying anything her practitioner noticed physical changes in areas where she would hold tensions.
This was such wonderful news to hear for both of us, proof that it is working and progressing positively for her.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
