Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client/Group progress summary

Client's sleep, nausea, shoulder pain (7/10) and L hip pain (6-7/10) is relatively stable. Client's depression med had been adjusted down by PCM a week ago hoping to resolve nausea, but client went back up in dose today as depression had been worse over the past few days, but better today. Also stated shifting our appointment from 1 pm to 3 pm was better with the nausea (usually wears off around 1 pm). Client has placed some art pieces at the local VFW and Mission BBQ, and is planning for a full showing of a collection at a local brewery.

Reflection and self-evaluation

Client forgot to have hand weights ready again. Client still hasn't found a yoga block, and his wife is a yoga teacher.
Adapted well to the regular changes with this client. Discussion about wrapping up the yoga therapy went well, with potential final session in 2 weeks after the holiday unless there's anything new the client would like to work on, with the usual instruction to reach out in between if anything comes up or anything new he'd like to work on.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
