Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

1. Three-part breathing daily for 5 minutes followed by alternate nostril for 5 minutes
2. Kirtan Kriya daily 5 minutes
3. Brahmari breath as needed
4. Positive affirmation daily and as needed
5. Gentle movement with breath 15 minutes, twice a week and when feel stressed and overwhelmed.

Activities

: The client has been diagnosed with chronic inflammatory response syndrome (CIRS). She initially had low energy and was fear of losing cognitive function. The asana and pranayama also focused on strengthening the muscles that facilitate respiration (diaphragm, intercoastal muscles as well as chest and abdominal muscles).

Client/Group progress summary

The client breathed much better today: no wheezing while in supine position (I offered a pillow under the head during savasana). She could hold her breath for several seconds after inhalation and had no problems with Ujayyi breath. The tandem standing is still a challenge for her. Her fatigue and cognitive function improved from the last assessment which was done 1 month ago (and almost two points improvement from the average scores assessed at the first session).

Reflection and self-evaluation

The client read the Yama and Niyama article that I shared with her after the last session. We discussed how these teachings can be adapted to in her daily life; some of which we have already talked about in the previous sessions. For example, applying Ahimsa, Satya and Isvara Pranidhana when communicating with someone who had opposite ideas/believes – find the common ground, avoid bringing up issues that will bring conflicts, cause resentment, and leave the outcomes in the hands of the higher power. We also discussed pranayama – recommending the client to observe the length of her inhalation and exhalation and increase it slowly; she can also extend the exhalation for 1 or 2 seconds longer than inhalation.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
