Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them. Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Homework assignment to client/group
We decided that until the next session she would pay attention to her breathing work on practicing breathing for 2 minutes every day.
Activities
The client had completed her intake from and we met online for the first time for a initial assessment.
She is a 58 yo female who was diagnosed with MS in August 2024 after having covid 3 months prior. She is very frustrated about her new situation. She likes to be in control and now her body is not allowing her to enjoy her usual routines and lifestyle. She is suffering from fatigue and decreased energy. She used to be very active and enjoyed traveling, now she does not like to make plans because very often she does not feel like doing anything because of the pain she is experiencing in the area of the trigeminus nerve and the heaviness in her right arm and shoulder. Cold often triggers her pain.
We started out with centering and a body scan to help noticing which area of her body is affected. She described that if she breathes into her nose and out if her mouth she experienced pain in the right side of her face, as the pain is triggered by cold air. We explored as to why she felt that she had to breath into her nose and out of her mouth, she states that she had practiced this with her PT. So she decided to pay more attention to her breath and what triggers the pain.
We continued with neck rolls, arm raises, side bends and chest openers
She states that she likes a more gentle practice because the yoga/pilates she had signed up for has cause her pain.
Client/Group progress summary
The client was very happy about this approach that is different from the approach her PT takes with her and she often is in pain after PT.
Reflection and self-evaluation
I was able to provide the client a safe space and she states that she feels comfortable with me and my approach so she wants to continue meeting with me and give yoga therapy a try.
Final Client/Group Report
After seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session
I will further explore what causes the client pain and what are her triggers emotionally and physically.
