Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

We've agreed that this will be our last session. I've given her a summary of the various asanas, hand exercises, breathing practices and meditation guidelines to refer to going forward. I also offered to meet with her again in the future.

Client/Group progress summary

All in all, I think the client has made good progress over these past few months. I'm grateful to see that she does appear more relaxed and aware of herself when she is feeling stressed or down. Her depression seems to have improved. I know she has the awareness, tools and practices to support herself when she finds herself facing tough circumstances and feelings.

Reflection and self-evaluation

I'm grateful to have had the opportunity to work with this client. While we didn't click and her progress wasn't as much as I had hoped for, I trust that I did my best and that she'll do her best with these practices going forward.
