Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Client/Group Goals

The client’s goal was to use yoga to support his overall physical and mental well-being. Based on the Global Health assessment, his overall well-being and quality of life remains the same (which is already good). I did not revise the goal, although I think that I should have done so (see what I would do differently).

Physical level

The physical level based on Global Health assessment is slightly worse, although this is not apparent during the session.

Energetic level

The energetic level remains the same.

Emotional

The emotional level is the same or slightly improved, mostly because the test results post-treatment shows no indication of the recurrence of cancer.

Intellectual / Sense of self

He identifies with his family. His life still revolves around his wife and daughters as well as extended family.

Spiritual orientation and support plan

He feels connected with nature and therefore, he and his family live in an area surrounded by trees and wildlife. Spiritual support plan is to take a walk either alone or with his family. I also have suggested walking meditation which can be done outdoor.

Additional Information
Feedback received from client/group, anecdotal or written

The client appreciates pranayama practice. He finds that breathing practice has been helpful in calming his mind and reducing anxiety.

Sample of homework given between sessions (after initial homework)

• Three-part breathing and alternate nostril breathing (5 min) and meditation (gratitude) 10 min
• Mindful practice – incorporate to daily activities (eating, brushing teeth).

What you would change with benefit of hindsight

1. I would collaborate with the client to revise his goal. Perhaps, using motivation interviews to understand the reasons for seeking a yoga therapist instead of going to a class.
2. Looking back, I would spend more time on subtle practices and give him articles on related yogic wisdom as I did with other individual clients or group sessions. Because he often gets distracted at the end of the session (daughter wakes up from her nap; his wife came home from shopping), I would start the session with subtle practices (breathing, meditation, guided imagery) first, followed by asana practice, savasana and brief (30 seconds) meditation.
3. Finally, I would follow up with him more often to remind him about home practice.

Questions, problems, areas in which you’d like more support

Do you think that PANAS assessment to better understand his emotion would be helpful in this case? He seems happy and positive, but I think that it would be useful to check.
