Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

check in, centering, awareness practice and opening chant - to calm and focus the body and mind, raise energy and allow the mind to turn inward. 2-5
Warm up; joint freeing practice and sun salutations - sessions 2 -4
Annamaya - Hatha, and yoga nidra (sessions 2-4) and restorative yoga (session 5)
Pranamaya - Dirgha Svasam, Nadi Suddhi and Brahmari
Manomaya - meditation and concentration on the breath
Vijnanamaya - Sutra 1.14 - the importance of a daily practice; SMTT - Four elements of stress management
(understanding, relieving, steadiness and coping); Sutra 2.33 switch negative thoughts to positive thoughts

Client/Group progress summary

Client is aware of triggers and the physical and emotional effects of stress on the body. Client has been using the breathing exercises covered in the sessions to reduce stress, specifically when traveling. The client is motivated in continuing the sessions.

Reflection and self-evaluation

Instruct client at the beginning of a restorative practice that they will be in each pose for a longer period and explain the benefits of staying in the pose for longer periods.

It is best to slow down then hurry through the sessions.
No matter how terrible I thought I did, the client always benefited from the practice.
I am enjoying my time.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
