Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

I made adjustments to our Asana sequence to focus more on chair yoga. She seemed to enjoy working with a chair more than on the mat. I also adjusted the balancing sequence to accommodate her injuries. She was always resistant to work on her balance, holding the belief that she's never been good at balancing. We unfortunately didn't make much progress in this area.

Client/Group Goals

Client has made notable progress with her goals through regular practice of her hand exercises and breath practices. She seems more resilient as well in light of some challenging events she experienced back in the fall.
No need to review / revise any of her goals. We simply adjusted specific practices due to car accident, which enabled us to accommodate her goals.

Physical level

Client reported overall improvement with the discomfort in her hands. She also seemed more relaxed in her body. The sense of bracing had softened a bit.

Energetic level

Energy definitely improved, seemed more grounded and content. I attribute this mostly to her reconciliation with her boyfriend.

Emotional

Client was somewhat more steady emotionally in spite of her ongoing stressors. She seemed more relaxed and accepting of her challenges and resolved to face them and take action.

Intellectual / Sense of self

Client seemed more grounded and sure of herself. Definite signs of resilience and acceptance.

Spiritual orientation and support plan

I don't know that there was much progress in this area. I do sense she experienced some more self-compassion and kindness with herself and all that she's been facing.

Client has been most grateful for our work together. She said it's made a big difference in her life in many ways. She even volunteered to submit a testimonial for my website.

Questions, problems, areas in which you’d like more support

I'd like to discuss what to do when you experience a seeming disconnect with a client. I often felt that our work wasn't making a difference for her, because she didn't seem engaged at times. Yet she kept showing up and expressing her gratitude.
