Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.
Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

There are no changes in the overall care plan, except for when the client was exposed to dust/toxin and did not feel well just before the session. She felt dizzy and unstable physically and emotionally. Therefore, I offered restorative yoga instead of adaptive asanas.

Client/Group Goals

The client's goal was to improve her balance and cognitive function, and she has achieved this goal. Although improved breathing was not her original goal, this was added to care plan to support pranayama kosha.

Physical level

The client gains more strength and flexibility. Her balance has improved; she can do the Tree pose without holding on to a char for 30 seconds or more.

Energetic level

The client's breathing pattern has improved; she can take a full breath and demonstrate abdominal breathing. Her energy level has also increased.

Emotional

She is calmer and does not feel anxious. Her cognitive function has also improved.

Intellectual / Sense of self

She is less reactive to stress and can think clearly.

Spiritual orientation and support plan

Her connection to nature and others become stronger. She now has a dedicated space for her spiritual practice. Walking meditation in the garden can be done when the weather is warmer. She has done this in the past and enjoyed it.

Additional Information
Feedback received from client/group, anecdotal or written

Nuch has offered me individual counseling on perspectives to avoid stress and resulting afflictions and suffering. Her instruction for development of “good” karma has opened to me a path for more positive impressions and recollections of my interactions and reflections. Her physical training routines with me have greatly improved my motor skills and balance. Nuch has been more patient with my progress than I have been with myself. I have recently learned that I can only regain what I have lost through illness by being persistent (regular & slow practices) and patient (don’t push too far or too fast) in my training, yet another “better karma” reflection.

Sample of homework given between sessions (after initial homework)

1. Three-part breathing followed by alternate nostril for 10 minutes.
2. Kirtan Kriya/meditation daily 5 minutes.
3. Gentle movement once a week. Include savasana, three-part breathing and meditation after asana practice.
4. Reading assignments, for example:
(i) Letting Go chapter of Jon Kabat-Zinn’s Wherever You Go There You Are
(ii) A Dog’s Quest to be a Buddhist by Gary Heads.

I would record guided imagery or meditation and share it with the client for her to practice while I was away for 1 month.

The mentor has been very supportive and provided guidance for the care of the client.

Absolutely. I have learned so much from working with this client. It warms my heart to see how far she has come from when we started working together.
