Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Checking in with her client about her mood, energy, sleep and concentration. She states that she continues to be sad, when asked about what brings her joy and prompting that in the past she liked being in nature, she states that this is what she would have said in the past but due to her limited mobility this is no longer possible. We talked about that she likes being on her balcony overlooking nature, she states that she can see how she still enjoys to connect to nature. She did state that her doctor made a referral for her to talk therapy. She states that her sleep is good and her energy is fair.Her concentration is ok but she still does not like reading too much but she also was not an avid reader before her stroke.
Centering with body scan and deep breathing.
Shoulder rolls, at the same time and with opposite shoulders like a swimming crawl motion.
She is able to use her right hand to guide her left hand and arm.
Started out with mobilizing her fingers individually and then interlacing her fingers. Raising her arms with inhale and lowering the arms with exhale.
Ringing both arms out to her middle of the body and then twist to the left and right with the head looking over the opposite shoulder.

Client/Group progress summary

She is making slow process. She is open to her doctor referring her to therapy and she states that she would try it.

Reflection and self-evaluation

I do think that I am patient with her process and I am not raising her anxiety by pushing her too much. I am listening to her needs.
I did ask her what she would like to work on, she states that would be anxiety. We talked about deep breathing and she will continue to do this as her home work.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
