Final Client/Group ReportAfter seeing your client/group (for at least 4 sessions including interactive intake)
Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

Yes , I realized that I had to take things a lot slower that initially anticipated.

Client/Group Goals

Her mood has improved , she has a different outlook on her life. She mobility has improved, she is able to move her left leg and arm more.
I realized that it is important to celebrate every accomplishment even if the steps are baby steps and meet the client were they are.

Report briefly on each Kosha belowProgress toward wellness or worsening reported by the client/group or that you observed in the following areas
Physical level

Client is making progress on a physical level, she is now able to raise her left leg which was completely paralyzed after her stoke in November 2023.She is also more able to move her left arm al little more. She is able to transition herself from the wheel chair to the couch. She is able to take a few steps without her leg brace.

Energetic level

Her energy level is increased , she is a bit more active physically and mentally , not as sluggish as she felt in the beginning. She has been getting out more and this gives her a sense of community. She also does not feel tired as easily as she used to.

Emotional

It took the client a long time to trust me and open up to me.She is able to share more and to reflect more on her current situation and how this makes her feel.

Intellectual / Sense of self

She likes to analyze things on an intellectual level, it is much more difficult her her to sense things , to listen to her intuitions .I do feel she is slowly coming around to that more and trust more into her own capacities .She has accomplished a lot since she had her stroke and she needed to adjust to a lot of very different situation.

Spiritual orientation and support plan

She does not believe in organized religion, she does enjoy being in nature, she is disappointed that she is not able to get out as much as she used to she does acknowledge that she finds it peaceful when she sits on her balcony and is able to watch nature.
