Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

She states that she wants to work on building a regular practice. She is using that breathing practice several times during the day and she feels that it is helpful for her.
When talking about her goals she states that she has created a vision board for herself and she is feeling positive about that. She wants to increase her level of activity and wants to go to her local yoga studio more. She likes a yin yoga and a relaxation class there.
She states that she will do chair poses we practiced together, like cat cow in the chair, shoulder rolls, arm raises and twist.
One goal she stated was to find her own way to deal with her MS and to navigate the health care system.

Activities

Checked in with my client about her pain. She states that her pain has changed. She states that when she lays down the pain in the area of the trigeminus nerve increases. Her sleep however has improved. Her energy level is fair. Her concentration is fair. Her mood is ok. She states that she wants to work on a positive mindset.We talked about the yogic concept of pratipaksha bhavana, of turning something negative into something positive. She stakes that she likes the idea. We also talked about the topic of tapas how steady efforts building the groves of a positive samskara.

Client/Group progress summary

She is making progress in the sense that she is trying different strategies for herself and is prioritizing her needs. She did create a vision board for herself and wants to follow the goals she created for herself. She did do part of her home work and she is working on improving her self care.

Reflection and self-evaluation

I am giving the client space to find her own path and she appreciates this approach. I am able to listen and adjust our practice to the clients needs.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

The next session will be our last session. The client wants me to help her create a brief chair yoga session, so she can do it on her own.
