| Homework assignment to client/group | She states that she did her breathing practice and the short yoga sequence and she went to her yoga studio a couple of times. |
| Activities | She states that she just got back from her 8th acupuncture session. She is not sure if it is helpful to her and sometimes the acupuncture itself is causing her pain. She states that she is trying her breathing practice and tells herself that this pain will pass. She was seeing a cardiologist and they did an ultrasound of her carotid artery and it shows narrowing on the right side. She is now wondering if her pain in the right arm and trigeminus area is caused by this. They also found that she has high cholesterol, and her blood pressure is normal. She states that her energy and mood are up and down depending on the day. Her sleep is good most of the time. |
| Client/Group progress summary | The client states that what helped her the most about our sessions together that she was able to integrate small practices in her day. She states that every time after our session together she was feeling so much better and she feels that I was really abele to motivate her and she was able to make small changes, like decreasing her coffee intake, and drinking green tea instead. She is integrating breathing practices in her daily life. |
| Reflection and self-evaluation | I was able to be listen to the client, and adapt to her needs. She states that she did enjoy our meetings a lot even though she was skeptical how that would work over zoom. I am happy to see the progress my client did make. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | Today was our last session and the client did provide me with feedback bout our yoga sessions together. We did create a brief yoga session, so she can continue to practice on her own. She states that she would like to keep in touch and update me on her progress in about 1 month. |
