Approval Notice
Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

I did slow down my pace and adjusted to her needs. She was trying out a lot of different avenues of care doing our time together and I walked along side with her on this journey.

Client/Group Goals

She was able to set goals for herself and slowly work towards those goals. One of her goal was to decrease her pain in the trigeminal area of her face and this was only partially accomplished. Is is more accepting of the pain, she is trying to breath through the pain and sometimes it works for her.

Physical level

Her physical pain in her face and and right arm went down a little. She did try out different levels of activity according to her pain level and energy.

Energetic level

Her level of energy improved some and she was able to spend a little more time with family and friends as well as trying out different yoga sessions in the community.

Emotional

She states that her mood improved and she always felt good after our sessions together. This motivated her to continue with our sessions and while she was late to our sessions in the beginning, she made it a point to be on time for our last 2 sessions.

Intellectual / Sense of self

She still struggles with her sense of self, she states that she spend so much time in her life educating herself and she wanted a professional life for herself. The diagnosis of MS made her question herself and she is asking herself the question of who am I when I am not working. Explored this question, especially in our last session together and she was open to at a later time to explore the yogic path for herself more.

Spiritual orientation and support plan

While raised in the catholic church she is not practicing in any organized religion but she is open to explore the yogic path for herself. Community of family and friends is also her important for her.

Feedback received from client/group, anecdotal or written

She states that she had a very good time doing our sessions together. She felt motivated and was thinking more positive about her situation.

Sample of homework given between sessions (after initial homework)

Explored the topics of tapas, pratipaksha bhavana. gave her a small chair yoga practice( cat -cow, chair circles, arms raises and twists)and breathing practice like three part deep breath and alternate nostril breathing .
Alos explored doing a gratitude journal , while she was initially interested she did not end up doing it.While working on goals for herself, she did create vision board for herself.
