Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

I made several adjustments to better meet the needs of participants, ensuring that the material was both accessible and effective
- Modifying Exercises for Different Abilities
- Providing Guidance on Home Practice
- I reinforced the importance of listening to their bodies and encouraged them to start with smaller, controlled movements before advancing.

Client/Group Goals

Participants demonstrated measurable progress toward the goals set at the beginning of the program. Through education and movement practice, they developed a greater understanding of bone health and the tools necessary for long-term management.

Physical level

Participants developed greater awareness of their skeletal health, posture, and movement patterns. Through weight-bearing exercises, balance work, and alignment-focused movements, they strengthened key muscle groups supporting bone health. Many reported increased stability and confidence in movement, while some experienced mild muscle soreness as they adapted to new exercises.

Energetic level

Breath awareness was integrated into movement, helping participants connect breath with movement. Deep breathing techniques were introduced to enhance relaxation and reduce tension. Participants found that breath awareness helped them feel more present and in control.

Emotional

Many participants initially expressed concerns about osteopenia-related limitations, particularly the fear of fractures and falls. Throughout the workshop, education and guided movement helped shift their mindset, replacing fear with confidence. Discussions fostered a supportive environment where participants could share experiences, ask questions, and feel empowered in managing their condition.

Intellectual / Sense of self

As participants gained knowledge about bone health, they developed a deeper understanding of how movement and mindfulness contributes to overall well-being. They became more attuned to their body’s signals, learning when to challenge themselves and when to modify or rest.

Spiritual orientation and support plan

By the end of the workshop, many participants expressed a sense of accomplishment and newfound appreciation for their bodies. Moments of relaxation and grounding exercises provided space for ease and inner connection. A greater sense of empowerment and trust in their ability to support their bone health contributed to an overall feeling of well-being.

Sample of homework given between sessions (after initial homework)

Home practice: Diaphragmatic breathing. Continue with engagement of muscles in mountain pose, W1, W2 & tree pose with slow breaths in and out. Holding for 30 sec +. Practice daily so the poses can become firmly grounded and rooted and thank our bodies everyday when we wake up. Continue to be aware of posture in daily life.

More Time for Q&A and Discussion – While we covered key information, allowing extra time for participant questions, experiences, and discussions might have deepened engagement and made the workshop feel even more interactive.

Yes, I enjoyed this workshop because it allowed me to educate and empower participants in a meaningful way. Seeing their confidence grow and their mindset shift around movement was incredibly rewarding. I also appreciated the opportunity to refine my teaching approach and explore new ways to support bone health through yoga and mindful movement.
