Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

When working with a client who has low back pain and small children, I made several adjustments to her care plan to ensure it was both practical and effective. Understanding that her daily routine involves frequent bending, lifting, and carrying, I incorporated strategies to support spinal alignment and core engagement during these activities. I introduced functional movements, such as squatting instead of bending at the waist, engaging the core when lifting, and using breath awareness to reduce strain.

The client has made meaningful progress toward her goals of alleviating low back pain and managing stress more effectively. Through consistent practice, she has developed greater body and breath awareness.

Physical level

The client has developed greater body awareness and strength, particularly in engaging her core and pelvic floor to support her low back. She has learned to squat rather than bend at the waist when picking up her children, reducing strain on her spine.

She uses diaphragmatic breathing while lifting and carrying her children, which helps stabilize her core and reduce unnecessary strain.

Emotional

She has become more mindful of stress-related tension and has started integrating short moments of breath awareness and gentle stretching during her daily routine. This has helped her feel more in control of her body and less overwhelmed by the physical demands of childcare.

She has gained a deeper understanding of how her daily movements affect her back pain and is making more conscious choices to prevent discomfort.

An area for improvement could have been providing more specific guidance on how to adjust her workspace or home environment to minimize strain, especially if she’s spending long periods doing activities like sitting. Offering practical, immediate solutions to these daily physical challenges could help her prevent pain before it arises.
