Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

To continue utilizing tools learned together as needed. Add in 10-15 min of gentle yoga movement and breathwork 3x/week until we meet back up.

Activities

Centering (5 min): Centering and grounding

Discussion and Check -In (10-12 min): discussed how things went for her during the holidays. My client was traveling all Nov/Dec so we spent some time talking about her travels and if anything came up for her during this time.

Breathwork (10 min): revisited 3-part breath and Nadi Suddhi. Talked a bit about whether she practiced any pranayama over the past couple months as well as how the breathwork felt in her body today.

Asana (20-30 min): Started in a seated position and moved into some gentle side stretches, forward folds etc. Came into table top, cat/cow, gate pose w/ lateral stretches. Came up to downward dog into a high plank and added in some core strengthening moves. Ended practice on the back coming into happy baby, spinal twist, waterfall and then onto back for final relaxation.

Relaxation (10 min): Body Scan

Closing (5 min): Discussed moving forward since my client is traveling again. She mentioned wanting to get back into a regular routine w/ sessions but said she can't commit until April.

Client/Group progress summary

It was good to meet w/ NZ after all this time and I kept the session basic returning back to breathwork techniques we've done already, and focusing on a lot of asana in our session today. She seemed happy to be back and seems committed to start regularly in April. This is her first year "retired" and her and her husband have spent the better part of the year traveling for vacation as well as visiting family so it seems that due to this, her commitment is not entirely there, but I think her intentions are strong.

Reflection and self-evaluation

I’m really pleased with the connection I’ve built with this client so far. From the beginning, our sessions have felt open and genuine, and we’ve developed a strong sense of trust. She seems comfortable in my presence, willing to share, and receptive to the practices we're exploring together. That level of comfort allows us to go deeper, both in conversation and in movement or breathwork. I’m looking forward to continuing our work, supporting her growth, and seeing how her journey evolves both on and off the mat upon her return.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
