Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

No pranayama, laughter yoga nor singing because the client's breathing becomes increasingly labored when focusing on the breath, instead I slow my own breath, speech and movements and she mirrors and relaxes, allowing her to breathe easier.

Check in - is about the client's comfort and to see if there is any pain; then we move on to what her day was like and what would she like to do with out time together and take it from there.

Movement - based on her energetic level we may do some limited joint freeing exercises.
Energetic - holding space until she relaxes and can breathe easier.
Emotional - Sutra 2:33, when distressed guide towards something positive using visualization or motivational interview.
Closing - Hand at the heart and letting her know it is time for me to go. I recap the session, ask if she is comfortable, did she have any questions, did she enjoy anything in particular. Is there any request for next visit, would she like to lay quietly or have the tv on?

Client/Group Goals

Progress in companionship and socialization with other residents of ALF.
The client is comfortable and well adjusted, no signs of distress.
Client is able to breathe easier when in a relaxed environment.

Physical level

Progress in mobility, mindfully transfers from wheelchair to bed; range of motion is good.

Energetic level

Progress in balancing energetic level to reduce labored breathing. I notice she is slowing her movements to be able to speak.

Emotional

Sharon's mood is improving, she has not cried nor complained about loneliness or missing her daughter. She and another patient held arms and shared smiles and pleasantries. She's made a new friend with similar backgrounds.

She is dealing with an attachment issue due to a recent gift.

Intellectual / Sense of self

Her posture has improved, she holds her head higher and lets me know her wants and needs.

Spiritual orientation and support plan

She remains in a peaceful state of mind; She has not mentioned a support plan nor shown any concern, however, hospice, daughter and ALF are her current support system. Hospice provides a chaplain visit weekly.

I would let the client know the services I am able to provide as a yoga therapist instead of introducing the practices as needed.

Questions, problems, areas in which you’d like more support

Am I doing enough for my client? I feel the comfort of this client is primary and whatever else shows up is met with compassion and ahimsa.

Did you enjoy your service?

Yes, very much and I look forward to continuing. Answering the questions above have made me realize that the little yoga therapy that she has received may have contributed to improving her quality of life. When I visit her neighbor's drop by to say hello and she feels special. She is heard, someone is listening.
