Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

I inviting PAM to reflect on the journey together by stating, “Let’s take a moment to reflect on where you were when we began, and what’s changed for you — physically, emotionally, or spiritually.”

I invited PAM to share what stands out or what she's learned about herself. I affirmed her strengths and growth, naming a few specific ones I've witnessed - her resilience, commitment, self-awareness. I then showed admiration for how she's stayed present with herself, even on the difficult days. I offered to create a sustainable Self-Care Plan by asking her which practices resonated with her the most. Was there any particular breath work for anxiety, gentle stretches for hip pain, or a YN? I offered to be a resource, and if she wants follow-up practices to let me know.

I then honored the closing this journey, of completing this chapter. “It’s been an honor to be part of your healing path. I trust you’ll continue to listen to your inner wisdom.” And closed with a relatively brief guided Yoga Nidra with a sankalpa to carry forward. For example, “I trust my body’s wisdom.” Or, “I am steady amidst change.”

We each lit a candle, meditated briefly, then chanted.

Client/Group progress summary

She's made spiritual progress, and I hope experienced emotional healing through this journey. We were able to summarize the last six months, sharing positive outcomes, lifestyle changes, body/pain/emotional awareness. How breath is with us wherever we go, and to remember to use it.

Reflection and self-evaluation

I've learned more about MI, the Yoga Sutras, how to pare down, prepare and present yoga information, be it philosophy, yogic paths, explaining postures and props online, etc 1:1, meeting people where they are, letting go of my ego, my attachments. It's been a journey for me as well as the client. Listening is more important than sharing one's personal experiences. Those can be saved for another time and place.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
