Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Focus: Self-inquiry, kindness, discipline, and study of self

Centering
• Review: Introduce one Yama (e.g., Ahimsa – non-harming) and one Niyama (e.g., Svadhyaya – self-study)

• Combine Dirga Swasam with awareness of inner reflection

Client/Group progress summary

The client is responding positively to the integration of physical movement and reflective themes. He demonstrates growing interest in yoga philosophy and a willingness to apply its principles toward cultivating patience and self-compassion. I recommended his continued use of breath-centered movement and a reflective practice.

Reflection and self-evaluation

This is the last session with the client. Breath work and body scanning were incorporated throughout to encourage mindfulness and present-moment focus. The client reported feeling “more relaxed and steady” by the end of the session and noted that practicing slower movement helped him feel “more in control” of his reactions.

At the end of the session, I noted that the client had already started integrating a lot into his life. He was then asked how he made that happen? He explained that he had incorporated the practices into his daily routine by setting aside a few minutes each morning for focused breathing, and by integrating selected standing asanas into his evening routine while preparing dinner.

I then asked the client to imagine himself in a few weeks and describe what his day might look like if these practices became part of his regular routine. He reflected thoughtfully and shared that he hoped to continue to start his mornings with a few minutes of breathing. He'd try to incorporate mindful movement into his daily activities, and use brief meditation or breath awareness practices to manage stress throughout the day.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
