Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Client/Group progress summary

Jasiri's stress level has decreased to a 3 in the past 4 weeks. We had not seen each other in 4 weeks due to Jasiri's cancellations on 9/11, 9/14, 9/18 and one week I was not available.
She has been doing home practice and finds the following useful: balancing, strengthening for balance, affirmations for sleeping and stress reduction, and joint freeing. She has not practiced breath awareness with extended exhalations.
She feels her balance has improved a little and she feels stronger but feels she needs to work on her flexibility and stretching.
She feels frustrated/negative self-talk with herself when she is unable to manage her time but is being more mindful about it. She is incorporating prep time to have everything ready and setting alarm to make it on time to her appointments/class. She tends to hold more than she releases. No one would know when she is overwhelmed because she will say she is doing fine and appears to be handling her responsibilities/obligations.
Finance is a bit cause of stress but all is good for now.

Reflection and self-evaluation

I changed the planned session to accommodate Jasiri's request for more stretching and asked if she felt comfortable doing a mat practice, she agreed and then said it was too advanced. I was guiding a gentle yoga ground practice. We immediately transitioned to the chair. It was not a big deal but threw me off and I had less time for a full session.
I will continue with chair practice sessions as planned.
I will suggest starting class at 6:15 instead of 6 going forward so that she does not feel frustrated for being late. She has been late to each session.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
