Physical

No issues with mobility, injury or pain.
Diagnosed with hypertension and hypercholesterolemia under control through prescription medications (Atorvistatin and Chlorithalidon)
Active, runs 6 miles on alternate days and works out at the gym on the days he is not running. In lieu of the gym he has attended a yoga class - he may substitute the gym with yoga.
During panic attack - increased respiration; tightness in chest; increased heart rate; and occassionally stomach ache
Isolation triggers his panic attacks and experiences claustrophobia during panic attacks only.
Measurement - Stress Scale 1-10 (low to high) = last week 8.5
Sleep cycle 11-7, quality is ok but lately wakes up to worry.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
