Physical

77 years old female client
Has gist in the stomach lining, may be cancer and will not metastasize. Requires removal because of growth. Started a year ago
Expecting surgery in the following weeks
Had uterine cancer and hysterectomy was done. Radiotherapy was given
Back spoke up loudly when tried to yoga after radiotherapy
Loss of flexibility and debilitating back aches
Overall health was good other than these recent encounters with health
Under continued observation post uterine cancer treatment
Not taking any medications other than occasional anti-inflammatory pills for back aches.
Able to get up and down from the floor
Cramping in legs while sleeping
Has a healthy diet
Experiences digestion issues which is taken care of

Experiences low energy level since last year
Experiences the most energy in the morning and low energy level in the evenings
The quality of sleep is great. Sleeps for 8 hours, waking up twice to use restrooms

Emotional

Has moderate amount of stress mainly due to health, concerns about future and relationship issues
Worries about her daughters, shared that one is not in touch and the other one attracts the wrong crowd and gets her second marriage.

Copes with stress by stretching and removing herself from the source

Feels sad or depressed for her other daughter as she seems to attract the wrong group. She gets her second marriage and hopes this marriage turns out well as her ex-husband was an alcoholic

Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.
Your care plan proposal should be approved by the mentor before session 2 if possible, or 3 if approval is delayed by mentor.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
