Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities: 
Theme - Rooted in strength, guided by the inner light
Focus: Grounding through the body, building strength with accessible poses, and calming the mind with Kirtan Kriya.

Seated Centering: sit comfortable, notice contact points with the floor and the seat
Body scan: Invite awareness from feet to crown, affirming safety
Breath Awareness: Slow diaphragmatic breathing (inhale 4, exhale 6)

Strength-building Seated warm ups: (i) Cat/Cow - connect breath with spinal movement; (ii) Utkatasana/chair pose: inhale arms up, exhale sit back - repeat gently x5 and hold on the 6th for a couple of breaths;
Standing warmups: (i) tadasana - root through feet, cultivate presence; (ii) Side Bends - strengthen obliques and open ribcage; (iii) Warrior II; (iv) triangle pose
Balance: tree pose
Mat: (i) bridge pose; (ii) child's pose
Backbend: camel pose
Forward bend: restorative
Inversion: legs on chair
Twist: prone twist with shoulder opening
Yoga Nidra: tense and release
Kirtan Kriya Meditation: silent repetition; Focus - visualize sound moving through the crown of the head and out into infinity

Client/Group progress summary

This past weekend he realized how intense the last couple months have been. He had a heavy feeling over that but feels he is ok. He acknowledged that if any changes are to be made it should be within. He has reduced a lot of activities that were not issential for him. He has significantly reduced his anxiety and attributes it to the reduction in interactions.

Reflection and self-evaluation

We took our time checking in. I asked Joe if he used any practice during the workday and he stated he was intentionally slowing down the transition from class to class allowing his students addiitional time to settle in. His students have noticed and are behaving different/better.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
