Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Focus: Grounding through the body, building strength with accessible poses, and calming the mind with Kirtan Kriya.
Seated Centering: sit comfortable, notice contact points with the floor and the seat
Body scan: Invite awareness from feet to crown, affirming safety
Breath Awareness: Slow diaphragmatic breathing (inhale 4, exhale 6)

Seated warm ups: (i) side flexion - connect breath with spinal movement; (ii) elbow to knee; (iii) pigeon (iv) extended side flexion (v) Inversion/twist: twist and reach opposite hand to foot, (vi) Utkatasana/chair pose: inhale arms up, exhale sit back - repeat gently x5 and hold on the 6th for a couple of breaths,
Standing tadasana - root through feet, cultivate presence
Balancing - tree pose (10 sec. each side)
Yoga Nidra - tense and relax (5 minutes)
Pranayama - extended exhale
Kirtan Kriya Meditation: silent repetition; Focus - visualize sound moving through the crown of the head and out into infinity (5 minutes)

Client/Group progress summary

Eugene was really tired, they had a false alarm last night. He has been practicing chair yoga at work but at home he has been supporting his wife. His stress level has been low except for last night.

Reflection and self-evaluation

I felt this was a good session. I added the breath of joy when I saw Eugene's energy level declining towards the beginning of our session. Eugene was unable to do the quad stretch in the plan, I could not come up with a modified quad stretch.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
