Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Intake Review, Orientation, and Introduction

Purpose: Establish rapport, assess movement, introduce chair-based yoga, and set expectations (NOTE: not interested in spirituality so reframe Pranayama as breathing, relaxation as a tool to let go of stress, and philosophy as a self-observation tool)

Check-In
• Review intake form and clarify goals
• Ask: “What would feel like success for you after four sessions?”
• Brief discussion of daily movement habits and fatigue patterns

Centering
• Seated, feet grounded
• Simple breath awareness without breath control
• Cue: “Notice where your body contacts the chair and floor.”

Chair Yoga (Introductory, Balanced)
• Seated posture education (neutral pelvis, chest lift without strain)
• Neck mobility: gentle rotations, side bends
• Seated Cat/Cow (small range, extension-focused)
• Shoulder rolls and scapular retraction
• Seated leg extensions with ankle pumps
• Gentle seated spinal rotation (upright, slow)

Pranayama (Reframed)
• Natural nasal breathing with paced rhythm
• Cue: “Let the breath stay smooth and unforced.”

Relaxation
• Seated body scan
• Imagery option: “Imagine the spine gently stacking itself, one segment at a time.”

Practical Philosophy Introduction (Jnana-based, reframed)
• Self-observation tool:
o Introduce the idea of noticing patterns without judging them
o Framed as: “We’re collecting data about how your body moves.”

Client/Group progress summary

Listened attentively, was very serious about the practice. Offered the opportunity to take some deep inhalations through the nose, breathing out of the mouth to relax but client appears uncomfortable.

Reflection and self-evaluation

She stated that her body felt less tense when we'd finished. "Yoga is new to me. I thought it was always on the mat. It looked so difficult, and my balance is poor ,so I wasn't going to try it. I think I can do this," she stated.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

To help her relax, during check-in I'll add some standing supported full body side stretches and arm swings to help loosen up the body and perhaps the spirit.
