Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Posture, Strength, and Breath Awareness

Purpose: Build foundational strength and postural endurance

Check-In
• Review body responses to first session
• Ask: “What felt helpful? What felt effortful?”

Centering
•. Stand with counter support for full body one-armed overhead stretches, side stretches, arm swings
• Seated grounding with slow exhale emphasis with eyes closed

Chair Hatha Yoga (Balanced)
• Seated spinal extension holds (postural endurance)
• Chair-supported chest opening with strap or hands behind chair
• Seated marching for hip flexors and core engagement
• Sit-to-stand from chair (functional strength, supported)
• Standing at chair: weight shifts and heel raises

Pranayama
• Lengthened exhale breathing (no breath retention)
• Cue: “Longer exhales help signal the nervous system to settle.”

Relaxation
• Seated
• Imagery: “Warmth spreading across the upper back and shoulders.”

Philosophy Tool (Karma Yoga, reframed)
• Action without overthinking:
o Framed as: “Doing small, consistent actions without waiting for motivation.”
o Apply to short daily movement breaks

Client/Group progress summary

The client seemed more relaxed at Saturday session.

MMP e-mailed me that it took effort to do the home practice so I sent her a printable message with posture pictures:

She commented that she printed and posted these on her monitors at work and at home.

Client was willing to close her eyes during centering then kept them closed for the rest of the session.

Reflection and self-evaluation

MMP emailed me that it took effort to do the home practice, so I sent her a printable message with posture pictures. She commented that she printed and posted these on her monitors at work and at home. She said that once she posted the cards, she was more likely to practice it. "They didn't take much time. My colleagues asked me what I was doing so I told them. Two asked if they could copy the cards so I emailed them the attachments to print!"

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

MMP appeared pleased to share her cards and yoga therapy experience with her colleagues. Before the next session I'll email her today's asanas with visuals so that she may have them handy. The visual prompts led to some casual social interactions with her friends.
