Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Purpose: Improve balance strategies and tolerance for sustained movement

Centering
• Standing stretches and arm swings to help relax client

Chair & Supported Standing Yoga
• Seated core engagement with arm movements

• Gentle standing back-body strengthening

Pranayama
• Coordinating breath with movement
• Cue: “Exhale during effort to support stability.” Used this cue during asanas as well.

Relaxation
• Seated relaxation with imagery
• Imagery: “A strong, upright structure that still feels easy.”

Philosophy Tool (Raja Yoga, reframed)
• Mental focus training:
o Introduced as attention practice rather than meditation
o Focus on one sensation (feet, breath, posture) for short intervals

Client/Group progress summary

She noted that she liked the attachments so I sent visuals again so that she could print and post them if she wished to do so. She appeared ready to begin and seemed to like the repetition at check-in, i.e. the question, then the standing joint loosening before we centered.

Is trying to follow the cues for the breathing exercises by taking her time, appearing more confident this session.

Reflection and self-evaluation

When asked if she were more aware of how she held herself, she said, "The reminders you send help me to be more aware. That's when I check-in. It's not a habit, though." She said she felt a little more energetic when asked, especially when she remembered to "move my body, even when I'm sitting down. " She stated that she is too sedentary and gardening (her hobby) helps her to move. When asked if she is walking her dog, she replied, "No, I'm not strong enough." She harnesses him to her golf cart to run her dog, and takes her into the large, fenced backyard while she's gardening.

She shared that printed the asanas for the last session but didn't really do them very much.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
