Adjustments and adaptations you made to your care plan,

The primary modification involved the use of hands-on adjustments to support body awareness & alignment. MJL demonstrated limited proprioceptive awareness, particularly during left arm movements. During active forward flexion & abduction of the left arm, he exhibited compensatory scapular elevation by recruiting the entire shoulder girdle.

Additionally, MJL displayed limited awareness of foot placement, affecting overall posture and balance during movement. Hands-on guidance was provided to facilitate proper alignment, improve proprioceptive feedback, & support safe movement patterns.

To accommodate his upper body weakness, some asanas were modified for safety & effectiveness. Session length was extended beyond the originally planned time to allow for careful guidance, repeated practice, and hands-on adjustments. These modifications supported improved proprioceptive feedback, alignment, and gradual strengthening, while maintaining a focus on mindful, controlled movement.

Client/Group Goals

The client demonstrated slow & steady progress toward the goal of supporting emotional well-being, stabilizing energy, & improving body awareness. He showed an increased willingness to engage in mindful movement & breath integration.

Through seated & supported asanas, gentle breathing techniques, guided relaxation, & body scans, MJL developed improved awareness of physical sensations, tension patterns, & energy levels. I adapted my approach using appropriate hands-on adjustments to support safe alignment, enhance proprioceptive awareness, & increase comfort in the postures. He reported feeling more grounded following sessions & is beginning to recognize how breath awareness supports emotional regulation & stress management.

Physical level

The client reported, and I observed, gradual progress toward overall wellness, particularly in proprioceptive awareness. Notable improvements were observed in left shoulder movements, foot placement, balance, and relaxation.

During sessions, MJL was prompted to practice left shoulder movements while observing himself in a mirror. This visual feedback allowed him to recognize that he was lifting his entire shoulder girdle rather than isolating the left shoulder. With this awareness, he was eventually able to adjust his movement, gradually engaging the targeted muscles more effectively.

Breath awareness also improved, though he continued to have difficulty integrating breath and movement smoothly. His breath was exaggerated, requiring verbal cues to either soften the breath or focus solely on movement until he could comfortably combine the two. Continued guided practice is expected to enhance coordination, proprioception, and mindful control of both breath and movement.

Energetic level

The client demonstrated increased awareness of his Pranamaya Kosha, the subtle energy layer associated with breath and life force, through focused pranayama exercises. Guided Dirga Swasam and mindful inhalation/exhalation practices allowed him to notice shifts in energy, areas of tension, and relaxation throughout the body. He responded well to verbal prompts, which helped moderate exaggerated breaths and maintain steady, controlled inhalations and exhalations. As he connected with his breath, he reported feeling calmer, more centered, and energized, and I observed improved grounding, postural stability, and muscular relaxation. The client recognized that engaging with his Pranamaya Kosha through breath not only supported physical awareness but also promoted emotional regulation and a sense of vitality, reflecting a growing integration of his energetic body into daily awareness and mindful movement practices.

Emotional

During the session, the client demonstrated increased awareness and regulation of his Manamaya Kosha, the mental-emotional layer of his being. Through guided breathwork, centering practices, mindful movement, and relaxation, he was able to notice fluctuations in mood, tension, and mental focus. Verbal prompts encouraged reflection on emotions and mental states during asanas and relaxation exercises. The client reported feeling calmer, more focused, and less reactive to frustration or stressors. Observations included improved attention, deliberate decision-making in movements, and greater emotional resilience. When asked, he agreed that engaging with his mental-emotional layer through yoga therapy supported both emotional regulation and the ability to observe thoughts and feelings without immediate reaction, contributing to his overall sense of mental clarity and balance. MJL noted that this level of self-awareness was new to him and had not been apparent before starting yoga therapy.

Intellectual / Sense of self

The client, who is approaching retirement, reported feeling uncertain and somewhat adrift, expressing questions about his next steps. He expressed interest in continuing to contribute through volunteer work in the same field from which he is retiring.

To support clarity and connection to his deeper purpose, staff introduced motivational interviewing questions, inviting him to explore values, priorities, and potential paths forward. Additionally, the client was guided through a simple Sankalpa setting exercise, focusing on a short, meaningful intention aligned with his authentic self. He engaged with the exercise, reflecting on personal goals and aspirations, and reported a sense of centeredness and emerging direction. Observations included increased focus, contemplative awareness, and a willingness to explore options beyond his previous routine.

Spiritual orientation and support plan

The client demonstrated an emerging awareness and said he was able to access and apply his inner knowledge and yoga therapy into everyday life. Through guided yoga therapy practices, including meditation, breathwork, reflective thinking, and Sankalpa setting, he could connect with his sense of purpose and values. He noted an increased clarity in decision-making regarding his retirement, greater calm during transitions, and a willingness to integrate insights from yoga (philosophy) into daily routines. The client engaged with practices that support alignment between body, mind, and spirit, fostering resilience, self-reflection, and a sustained sense of meaning and inner guidance. MJL voiced a desire to continue yoga therapy following his trip and expressed pleasant surprise at the positive effects he experienced from our sessions. He noted improvements in his overall well-being, reinforcing his motivation to maintain these practices in daily life.

MJL shared that he looks forward to continuing yoga therapy after his trip, noting he was pleasantly surprised by the benefits he experienced. He reported feeling physically lighter and mentally calmer following sessions, highlighting improvements in body awareness—particularly in his left shoulder movements and chest movements (breath)—and appreciating the balance exercises. He expressed that practicing the three-part breath increased his sense of calm, and he enjoyed sharing the technique with his wife, practicing it together at home. Overall, he observed enhanced well-being, which strengthened his motivation to maintain these practices in daily life.

It's natural for your mind to wander during meditation. The key is to gently redirect your attention back to your chosen focus without judgment. Start with shorter sessions and gradually increase the duration as you become more comfortable.

1. Set timer for 5-10 minutes. When the timer buzzes, return to present moment.

2. Support yourself in a supine position, practice centering by relaxing your body using a body scan, clear your mind letting go of thoughts, then breathe with focus.

3. Notice rise and fall of each breath. Follow the full inhale and exhale for 10 cycles of breath. If thoughts show up (they will) or your mind starts to wander, see if you can gently return to the rise and fall of your breath. Returning as often as you need to, to be in this moment.

OR

In a Supported suppine pose:

1. Shoulder flexion (using other hand for support) to shoulder height, punch up (repeat)
2. External shoulder rotation (with/out dowel)
3. Snow angels
4. Side twists with bent knees

What you would change with benefit of hindsight

When working with a client in the future, I would spend additional time before each session centering myself to build confidence and presence. I would plan for a full 60-minute session, recognizing that engaged clients often wish to remain active for that duration. I also aim to deepen my understanding of how to introduce concepts of inner awareness or personal reflection to clients who do not identify as spiritual, using language that resonates with their beliefs. During intake, I would incorporate motivational interviewing questions to explore their values, perspectives, and sources of meaning, allowing me to tailor practices in a way that feels relevant and accessible.

Questions, problems, areas in which you’d like more support

The type of support I need involves guidance in managing self-doubt and moving beyond feelings of imposter syndrome, so I can approach each session with confidence and clarity. I also seek strategies to ensure that the yoga therapy sessions I provide are impactful and tailored to the client’s needs, even within the constraints of a limited number of meetings. This includes support in prioritizing essential practices, maintaining a clear therapeutic focus, and effectively adapting interventions to maximize client benefit in a condensed timeframe.

I find my service deeply fulfilling. It is incredibly rewarding to witness clients actively engage, explore new skills, and share their experiences and achievements with me and others. Observing their growth and progress not only reinforces the tangible impact of my work but also inspires me personally, affirming that each session contributes positively to their well-being. This work reminds me that meaningful change happens one person at a time, and that supporting someone’s journey—emotionally, physically, and mentally—is both motivating and gratifying.
