Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Activities

Centering.
Joint Freeing movements - Shoulders, head/neck, Cat/Cow.
Chair Asana (modified) - Sun Salutation, Standing Half-Locust, Seated Cobra, Seated Forward Bend, Seated Side Bends.
Seated body scan.
Guided Meditation with silence.

Client/Group progress summary

Client has been able to practice non-attachment with other persons problems. Client reports that she has been able to re-direct and detach emotionally, and without feeling guilty about not taking on other people’s issues.

Client reports that she does her meditation and asana practice every morning.

During the 5th session, client received shadow work therapy and Client reports that she had an amazing time doing the shadow work – it was a great release.

Reflection and self-evaluation

I would like to be able to have more fluency in the sessions – in that I wish the thoughts would come to me more easily as to how I should respond sometimes.

Note: As instructed by Sue Evans, on July 8th: all of the minutes for session 1 through 4 (461 minutes total), plus session 5 (150 minutes total) have been included in this Individual Session Report for a total of 611 minutes. There was nowhere else to put the minutes of sessions 1 - 4, and I was instructed to put it into the Individual Session report.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Next session will be our 6th session, and we will meet after the client’s birthday which is coming up. The plan is to see how the client is doing with her determination to cease smoking during her birthday. To talk about her progress with noticing internal stressors, attachments, aversions. To review these teachings, and to see what the client wants to work on.
