Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

Reviewed asana she can do at home: Chair - baby cobra, forward fold, twists; Standing - cow/cat, wide forward fold, dynamic twists.
Breath of Joy for energy and in place of caffeine

Activities

Reviewed breath of joy (she had some questions/confusion).
Discussed prayer/meditation/ritual and designing an altar for ritual space.
Discussed effects of cortisol/stress response and using movement (asana), breath, and meditation to reset the nervous system.
Discussed the use of pratipaksha bhavana (she had questions about denial or suppression of the "dark cloud" of negative thought).
I took her through a guided meditation with body mapping followed by chakra meditation with the sounds of a steel tongue drum.

Client/Group progress summary

Client is making progress, especially after all she learned in cardiac rehab. She is doing very well with following a healthy, mostly vegetarian diet. I reminded her how far she has come since before cardiac rehab.

Reflection and self-evaluation

I enjoy working with this client. When I first worked with her a few years ago as a counseling client, she was a mess physically and emotionally. I feel pleased seeing her do well, although I know her progress is not about me. She has finally owned the fact that her health is in her own hands - and she's doing the necessary work!

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
