Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Adding short stretch/breathing breaks during her work day; adding an evening stretching routine before bed.

Activities

Discussed issues she is having with her co-workers that cause stress. She doesn't want to spend more than one day a week at work, yet she has trouble working at home. She decided she can start working in an empty meeting room at the YMCA where she goes to exercise.
Discussed her daily prayer/meditation routine which is very solid in the mornings.
Discussed the meaning of the word Yoga - union, especially union with God.
Did a routine of chair asana followed by supine floor asana with strap stretching.
Took her through the guided meditation, "Relaxation Script to Cope with Problems" from SMTT manual, p. 192

Client/Group progress summary

Client is doing well with her homework assignments and staying committed to her exercise and meal plans. She has lost 28 pounds since beginning the cardiac rehab program in January.

Reflection and self-evaluation

I will like I'm doing well working with this client. I look forward to her coming for her sessions. We seem to connect well. She trusts me and can be very open in our discussions.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Will go through the gunas questionnaire with her. "Mental Constitution According to the Gunas" from Yoga of Recovery elective module. (Client is in eating disorder recovery and deals with anxiety and depression at times.)
