Initial Intake & Assessment - previously approved in old platform with mentor notes on care plan.

Activities

Client is a 59-year-old caucasian cisgender female diagnosed with gastroparesis and nonalcoholic fatty liver disease, as well as degenerative discs with bone spurs in her cervical spine. In conversation not reflected on the intake, client mentioned several abdominal surgeries over the past almost 20 years, including an exploratory surgery, removal of ovarian cysts, uterine cysts and fibroid tumors, gallbladder removal, surgery for urinary incontinence - and treatment for an abscess inside of sutured area, which took over 6 months to heal. In addition to experiencing pain through the jaw, neck and shoulders daily, the client feels discomfort in her right knee, more inside/medial region. She takes medication for IBS, menopausal hormone management, and high cholesterol. She was diagnosed with PTSD in 2019 and feels she also has undiagnosed depression and anxiety. She notes that relaxing, getting enough sleep, and exercise/movement improve her experience of pain.

She quit smoking 30 years ago, but does use marijuana in gummy form and drinks alcohol, approximately 3 beers or glasses of wine/week. She is aware of foods which aggravate her system and cause discomfort, but struggles with still consuming them regularly, and feels that binge eating is an issue for her.

She describes her stress level as "moderate," and feels consistently fatigued and depleted of energy. She holds her breath and often her breathing is shallow. She identifies sleep deprivation and poor sleep as current life challenges, along with caring for her partner with Parkinson's, working long hours, financial issues, and feeling responsible for/like she must "fix" the circumstances of family members. She feels a "need for control" but feels unable. While she sleeps almost 8 hours/night she does not wake refreshed, and struggles with disrupted sleep due to pain and night sweats.

She finds joy in spending time with her partner, children and grandchildren; her job; listening to music. She also loves serving others. If she could change something in her life it would be to "slow down...sleep good."

Client's self-identified goals are relaxation, "energy healing," and improving flexibility. As discussed in client plan attached, talked with her about exploring and adding practices for self-care across all her koshas - with a focus on gentle movement of her body to encourage better movement of her digestive system; pranayama practices to increase parasympathetic activation and ANS balance; incorporating the Raja yoga practice of pratipaksha bhavana to shift her thoughts towards what is positive and healing, to also support increased non-attachment/vairagna of what is not hers to control or carry.
