Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

Client will implement working from home. After her two trips out of town, she will work from the YMCA. She will go there to lift weights, then work virtually, then swim before going home.
Cardiac rehab recommended an hour of daily meditation. She has not been doing this, but wants to work on small meditation sessions spread throughout the day.

Activities

Discussed her challenging, stressful week with incidents at work and at home, plus her upcoming court date for divorce, child support, etc.
Asana: reclining hamstring stretches. hip stretches, and twists.
Pranayama: slow, deep breathing, and breath awareness.
Guided meditation: body scan, sensing "opposites" in the body, meditation using the symbolism she shared last session, especially trees/woods, water, and turtles.

Client/Group progress summary

Client acknowledges she has been "backsliding" a bit by not getting enough exercise, and by packing her schedule too full for her own health. She awoke feeling dizzy and lightheaded today. As she did in cardiac rehab, she will write all her activities on index cards, then divide them into things that must get done, things that aren't as pressing, and things she can drop from her schedule for now. This will allow her to make the necessary time for her selfcare activities. She states she is still doing well with her diet.

Reflection and self-evaluation

I enjoyed conducting the guided meditation today. She stated it helped her feel so calm and relaxed. We talked about how it resets the nervous system, and stops the "snowball" of stressors building on each other. I hope the client will make the changes she needs to make to keep prioritizing her health.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Client will be out of town next week. When she returns, we will do chair asana if she's feeling up to it. We will also review the handout on Raja Yoga, and review breath work.
