| Homework assignment to client/group | Homework for PFS-C002 after intake and assessment session on 03/05/2024. • Timed Up and Go. To facilitate smoother movement and make standing up more accessible. • Pelvic tilts. To increase spinal mobility. • Breath practices. To increase breath capacity, relieve stress, and lighten mood. |
| Activities | As per care plan ~ Awareness practice. Toe touches: front, side, back, together. Pelvic tilts, seated cow/cat, down dog at the wall. Relax and lighten up posture awareness and adjustment. Timed up and go. Breath practice: ‘Ah’ breath, extended exhale, bhramari. Guided relaxation, concluding with quiet instrumental music. Inspiration from role models such as Michael J. Fox. |
| Client/Group progress summary | Client’s concentration has improved, he has greater stamina, self-reported: his mood is uplifted at the end of sessions. He is more aware of his movements, thoughts, and emotions. His is slightly more tolerant of self-reflection. His standing posture and standing balance are about the same as when we began working together. The Lighten Up word cues are very effective in transforming his posture to a more upright stance with greater axial extension, relieving his hyperkyphosis. Transitioning from seated to standing is about the same as during our initial session. |
| Reflection and self-evaluation | Cut back on number of practices per session to give more time for self-assessment, sensory awareness, and transition between practices. Include a broader range of simple calming and energizing breath practices. Encourage client to express himself more without peppering him with questions. |
| Final Client/Group Report | After seeing your client/group (for at least 4 sessions including interactive intake) Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report. |
| Plan for next session | Continue with practices in care plan. Encourage self-reflection and self-compassion. Revisit ideas for more social engagement, possibly volunteering to become active in the local community. Find ways for the client to connect to a higher power and his ever-present light within. |
