Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Homework assignment to client/group

Homework for PFS-C002 after intake and assessment session on 03/05/2024.

• Timed Up and Go. To facilitate smoother movement and make standing up more accessible.
o Three to four times a week. Add to your workout routine where possible or in the afternoons.
o Reference the printout from the CDC.
 From seated, stand up, walk 10 feet ahead, turn around, then return to your seat.
 Repeat three times.

• Pelvic tilts. To increase spinal mobility.
o Daily. Could do when having afternoon coffee or any time of day.
o Seated on a chair with a firm seat. Tilt pelvis forward and back across the sitting bones.
o Repeat for three sets of five forward/back tilts.

• Breath practices. To increase breath capacity, relieve stress, and lighten mood.
o Daily. Maybe in the morning or when preparing to work out.
o Inhale through the nose. Exhale through the mouth making the ‘Ah’ sound.
o Repeat for five breaths.
o When you feel uneasy, unsettled, or anxious/agitated.
 Bumble bee breathing. Inhale through the nose, exhale and make a humming sound. Continue for 10 or so breaths, or as long as it feels right to you.

Activities

As per care plan ~ Awareness practice. Toe touches: front, side, back, together. Pelvic tilts, seated cow/cat, down dog at the wall. Relax and lighten up posture awareness and adjustment. Timed up and go. Breath practice: ‘Ah’ breath, extended exhale, bhramari. Guided relaxation, concluding with quiet instrumental music. Inspiration from role models such as Michael J. Fox.

Client/Group progress summary

Client’s concentration has improved, he has greater stamina, self-reported: his mood is uplifted at the end of sessions. He is more aware of his movements, thoughts, and emotions. His is slightly more tolerant of self-reflection. His standing posture and standing balance are about the same as when we began working together. The Lighten Up word cues are very effective in transforming his posture to a more upright stance with greater axial extension, relieving his hyperkyphosis. Transitioning from seated to standing is about the same as during our initial session.

Reflection and self-evaluation

Cut back on number of practices per session to give more time for self-assessment, sensory awareness, and transition between practices. Include a broader range of simple calming and energizing breath practices. Encourage client to express himself more without peppering him with questions.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.
Plan for next session

Continue with practices in care plan. Encourage self-reflection and self-compassion. Revisit ideas for more social engagement, possibly volunteering to become active in the local community. Find ways for the client to connect to a higher power and his ever-present light within.
