Your care plan should be approved by your mentor, with any amendments they suggested, prior to your remaining Yoga Therapy sessions.
Care PlanOutline should be a practice adapted to the needs of that client/group, including:
  • Check in, centering, balanced hatha yoga set considering contraindications, relaxation (with imagery as appropriate),
  • balanced pranayama considering contraindications, meditation/centering.
  • Please include at least one suggestion from Karma, Bhakti, Raja, or Jnana Yoga tailored for this client/group.
  • Over time, we want to see something from each branch, selected, adapted and re-framed appropriately. Tools from each module should be used (not on each client/group – but overall)
The outline should show the sequence of practices as you plan to offer them.

Please remember practicum is a learning experience. You’ll learn more from sharing what’s accurate than from what might “look good”. Things you did well, not so well, problems and questions are all valid and useful tools to teach you. We can’t serve you to become the best clinician you can be if you don’t share your challenges and mistakes. Success is anything from which you learn. You can continue to add Session entries after submitting this Final Client/Group Report.

Adjustments and adaptations you made to your care plan,

• Adjustments and adaptations you made to your care plan. Added a different dynamic standing balance sequence that the client really likes. (See Exercise 7.6 below) Explored standing twists to address tendency to turn en bloc. Added supported chair pose at the wall to strengthen legs and counter forward head posture.
The client was fatigued at the start of the first of the session. I switched the awareness practice to after check-in with client in a supported supine position to rest. The client said he prefers fewer words and enjoys music at the end of the session. I shortened the guided relaxation and follow it with quiet solo guitar music, then silence.

Client/Group Goals

Balance, posture, ability to stand from seated.
o Any progress towards goals. The client has a hard time with coordination and proprioception. He also has some cognitive decline, so focusing and intentionally making change are not easily accessible to him. Even so, his concentration has improved. He can remain engaged in activities longer. Sustained focus helps him to balance.
o Review and revision of goals. Client’s original goals are still of interest to him and applicable. Client would like to work on strengthening his legs and do some breathwork.

Physical level

Client says his body feels “more fluid” at the end of YT sessions. The client’s cow/cat movements are smoother, more rhythmic, and easeful with less head bobbing.

Energetic level

Client appears more rested and alert after our sessions. From client’s observation, his tremors stop during the YT sessions and he’s able to relax at end of session. Linking breath and movement is beginning to be more intuitive for him.

Emotional

Client acknowledges some shift in his mood after YT. His mood seems lighter than during the initial intake session. He is frustrated with his loss of coordination and disease progression.

Intellectual / Sense of self

The client appreciates learning how the practices he engages in relate to specific symptoms of PD. The sessions can be mentally fatiguing for him.
Sense of self. No change.

Spiritual orientation and support plan

Gave the client a printed inspirational Michal J. Fox (MJF) quote and image to remind of MJF and encourage to him. Suggested that the client display the quote/image in a place he sees it often for inspiration. Followed up regularly regarding quote. Client found it hopeful and motivating.

Additional Information
Feedback received from client/group, anecdotal or written

Client finds sessions lift his mood, reinvigorating, challenge his thinking, relieve upper and lower back pain, improve his posture, help him to relax and destress.

Seated: pelvic tilts, cow/cat, neck stretches, joint freeing series, standing toe taps, timed up and go sequence without timing, humming breath, inspirational quote reflection, supine relaxation to soothing music.

What you would change with benefit of hindsight

Incorporate his love of bicycling into the sessions. May be able to tie it in to spirituality for this client.
